Food hygiene license, referred to as hygiene license, is a hygiene license issued by units and individuals engaged in food production and marketing activities after being examined and approved by the food and drug administration. Application for Health permission;
A copy of the notice of name pre approval;
Copies of the identity certificate and other supporting materials of the legal representative or person in charge (including resolutions of the board of directors, articles of association or appointment documents, and copies of the identity card);
Use certificate of food production and operation site (house property right certificate or lease agreement), contract or agreement;
Layout plan of production and operation site;
The health management organization and various health management systems of the applicant;
The health examination certificate of the employees (with the official seal of the physical examination institution) and the health knowledge training qualification certificate of the employees (with the official seal of the training institution);
Employees' health knowledge training questions (one for each person, which must be answered manually);
"Three certificates" (hygiene license, business license, product inspection certificate or inspection report) of purchased raw materials (such as rice, flour, oil and meat) and meat inspection and quarantine certificate;
A copy of the agreement (contract) on the treatment of swill or waste edible oil.
According to the "measures for the administration of food hygiene license" issued by the Ministry of health in December 2005, any unit or individual engaged in food production and marketing activities shall report to the administrative department of public health and go through the application procedures for the hygiene license according to the regulations; only after the examination and approval of the administrative department of public health can they engage in food production and marketing activities and bear the food hygiene responsibility for food production and marketing.
The administrative department of health of the local people's Government shall abide by these measures and issue hygiene licenses to food producers and operators.
The hygiene license for production activities of food additives, health food and new resource food production enterprises shall be issued by the provincial health administrative department.
