1、 Types of insurance for individual social security: basic endowment insurance and basic medical insurance

Basic endowment insurance + local supplementary endowment insurance 19%,
Basic medical insurance + local supplementary medical 8.5%
3、 Information to be provided for individual social security participation
（1） Fill in and submit the declaration form for individual payment of social insurance and the contract for entrusting a bank to collect social insurance premium;
（2） A copy of my ID card and household register (check the original copy);
（3） For the first-time insured persons who are employed or transferred to Shenzhen, a copy of the transfer order (or recruitment form) shall be provided (check the original). For those who are registered permanent residents and whose female are 35 years old or male are over 45 years old, the endowment insurance status certificate issued by the original social security institution shall be provided;
（4） A copy of the account name and account page of my savings bank passbook opened by one of China Construction Bank, Bank of China, Agricultural Bank of China and industrial and Commercial Bank of China;
（5） When applying for individual insurance payment, the employees who already have "XX City labor security card" or "XX city employee social insurance card" must examine and verify "Shenzhen City labor security card" or "Shenzhen city employee social insurance card"; those who apply for "XX City labor security card" need to provide the digital photo receipt of the second generation ID card online museum recognized by the municipal public security organ.
4、 Insurance procedures
（1） Collect and fill in the declaration form for individual payment of social insurance in a certain city and the contract for entrusting a bank to collect social insurance premium at the individual payment window of social security institutions;
（2） Hold the contract of entrusting the bank to collect social insurance premium and affix the official seal at the Bank of deposit;
(three) to take the information required by the insured, the declaration form of social insurance payment for individuals and the contract for collecting social insurance premiums by the entrusted banks, and to go to the personal payment window of the social security institutions in the registered residence area for the insurance procedures.
（4） Those who need to apply for the "XX City labor security card" must come to apply for it in person, and provide a copy of the ID card (check the original) and the digital photo receipt of the second generation ID card networking Museum recognized by the municipal public security organ.
5、 Other precautions
The insured shall deposit enough social insurance premium in the bank collection account before the 21st of each month to prevent collection failure. If the collection is unsuccessful for three consecutive months, the social security institution shall stop the insurance for it. If it re participates in the insurance, it must declare again.
