After the continuous fermentation of the network society, the event of the maternal jumping off the building in Yulin, Shaanxi Province has become a hot topic of the whole people's attention. Because the family refused to sign the informed consent, the maternal jumped off the building in despair and committed suicide, which is sad! The hospital even announced the monitoring screen of the maternal kneeling twice to the family members to request the caesarean section in the early morning. It is estimated that the people who have seen it will be filled with indignation!
On August 31, 2017, Ma XX, a pregnant woman in the second ward of Obstetrics and gynecology in Suide Hospital District, died of jumping off the building. After the incident, our hospital, the superior competent department and the public security department attached great importance to it. An incident investigation team was set up at the first time to fix the evidence and carry out the work of cause of death identification and aftermath treatment. Three communication meetings were held with the family members of the deceased successively to express their condolences and sympathy, and to report the progress and preliminary conclusions of the investigation. In response to the Internet rumors, our hospital issued the relevant situation statement on September 3, and actively accepted the media interview and supervision. In order to further clarify the facts and restore the truth of the event, the following are the doubts about the relevant situation and the events of public concern:
I. Basic Information
Ma XX, female, 26 years old, ID No. 6127xxxxxxxxxx3325, born in zhangjiafeng village, Jizhen Town, Suide County. At 15:34 on August 30, 2017, the puerpera was admitted to the hospital with the main complaint of "41 + 1 weeks of menopause requiring hospitalization for labor". Preliminary diagnosis: 1. At 41 + 1 weeks of gestation, the first child is ready to give birth; 2. The risk of dystocia in vaginal delivery is high because the fetal head is too large (color ultrasound indicates that the biparietal diameter is 99mm, generally the biparietal diameter of full-term fetus is not greater than 90mm), the doctor in charge has explained the situation to the pregnant women and their families for many times, and suggested that the cesarean section should be carried out to terminate the pregnancy, and the pregnant women and their families have clearly refused, and insisted that oxytocin should be used to induce uterine contraction and vaginal delivery《 The informed consent of the puerpera shall be signed to confirm the requirement for the smooth delivery.
On August 31, 2017, at about 10 a.m., the puerpera entered the waiting room. During childbirth, due to pain and restlessness, the puerpera left the waiting room many times and asked the family for cesarean section. The doctor in charge, the midwife and the department director also proposed cesarean section suggestions to the family, which were all rejected by the family members. Finally, because of the intolerable pain, the parturient woman lost control of her mood and jumped from the building. The medical staff shall rescue the patient in time, but the rescue is invalid due to the serious injury.
II. Rehabilitation process
On the evening of August 31, our hospital sealed the maternal medical records, surveillance videos and other relevant evidences for the first time.
On the morning of September 1, our hospital set up an investigation team to ask the relevant parties to fully cooperate with the public security department in the investigation, and designated a special person to be responsible for pacifying the family's emotions and understanding the demands.
On the morning of September 2, our hospital held a discussion with the family members of the patients to answer their questions about the reasons for the patient's jumping off the building and the diagnosis and treatment process.
At 9 a.m. on September 3, the person in charge of Suide County Public Security Bureau came to the hospital to hold a symposium among the police, the hospital and the family members, to inform the preliminary investigation conclusion of the death of the parturient by jumping off the building, and to suggest that the family members of the parturient should solve the objection through legal means such as litigation.
III. Explanation of main disputes
(I) cause of maternal death?
The public security department has issued a written investigation conclusion: excluding homicide, the puerpera committed suicide by jumping off the building.
(2) who refused to have a cesarean section?
1. The couple of the parturient sign the informed consent of the parturient in hospital before delivery, sign and confirm the willingness of the parturient according to the fingerprint;
2. The nursing record sheet records three times of rejection by family members during the labor process;
3. Communication between the mother and her family members in the surveillance video was rejected.
(3) why does it have to be signed by the family members?
The mother signed the letter of authorization, authorizing her husband to be fully responsible for signing all relevant documents. When she did not withdraw her authorization and there was no life-threatening emergency (the blood pressure and fetal heart rate of the mother recorded in the delivery process) without the consent of the authorized person, the hospital has no right to change the mode of birth.
(IV) is there any loss of custody for medical staff?
1. The parturient is an adult with no history of mental illness and has full capacity of behavior. Even in the hospital waiting for delivery, she has no right to restrict her personal freedom;
2. Generally, the parturient will have a few hours of labor, and most of them will get up to talk or walk with their families outside the delivery center;
3. At the time of the accident, there were 5 parturients in the waiting room. The midwives on duty picked up the newborns in the delivery room, and the second-line midwives inspected the progress of each parturient in the waiting room;
4. The woman had been out of the delivery center for many times to communicate with her family members, so when she left the waiting room for the last time, the midwife did not expect the woman to jump into the spare operating room opposite the waiting room and die.
(V) why are the windows of the hospital without protective facilities?
1. The height of the incident windowsill is 1.13m, which is in line with the building safety code, and there is no possibility of accidental falling;
2. Article 28 of Chapter II of the fire protection law stipulates that 'no obstacles affecting escape and fire fighting and rescue shall be set in doors and windows of densely populated places'. The fourth paragraph of the seventh chapter is clear: 'the densely populated place refers to the public gathering place, the outpatient building and ward building of the hospital, the teaching building and Library of the school...... Places for tourism and religious activities, etc. '
