Why does crayfish cause rhabdomyolysis in Shanghai?

On July 30, three diners set up a table at Zhangyang store, Shanghai xiaopang store, including a jin of gourmet lobster, one for the supreme frozen lobster, one for the garlic flavored lobster, one for the thirteen flavored lobster, one for the snail and one for the beer. The total cost was 1156 yuan. In the early morning of the next day, the three people had symptoms of muscle ache and restlessness.

According to the blood test results of Shanghai Oriental Hospital, the values of creatine phosphokinase isoenzyme and myoglobin in the three people on the table were soaring, which should be diagnosed as' Haff disease 'rhabdomyolysis syndrome.

Another crayfish shop with similar accidents is the "monster lobster" in Jinqiao, Pudong. At present, the three diners in Shanghai xiaopang and the other two diners in monster lobster have been treated.

Caixin's reporter learned that the reaction of "Shanghai xiaopang" and "monster lobster" was that there were many customers that night, but only one table had problems. The current situation does not show that rhabdomyolysis is caused by eating crayfish.

The crayfish in the two stores involved were all from Jiangyang aquatic products wholesale market in Baoshan District, and Shanghai xiaopang presented a quick test report issued by Jiangyang market test center to the local media. Crayfish in this market has a certain share in Shanghai market.

According to the literature, rhabdomyolysis syndrome has been found for more than 80 years since it was first found in 1924. The disease occurs in the Baltic, Mediterranean, the United States and Brazil. It is mostly related to food such as Buffalo, cod or crawfish, but the number of cases is very small.

Cases of rhabdomyolysis caused by crayfish consumption have been reported in China, all of which occurred hours to more than ten hours after crayfish consumption. The clinical symptoms of the disease are: persistent muscle pain, mainly manifested as systemic or local pain, and individual cases with low back pain, fatigue, chest distress, respiratory pain and other symptoms.

Some viruses, lipid-lowering drugs, beta; 2 receptor agonists, amphetamines, drugs and other drugs that cause muscle damage can also cause muscle lysis. After the membrane of muscle cells is destroyed, the contents of muscle cells are released into the blood, which includes a large number of myoglobin. Because myoglobin needs to be discharged from the body through the kidney, myoglobin itself is a 'big man', so it is easy to block renal tubules and affect renal function. The symptom is lighter can produce haemoglobinuria, serious still can cause acute kidney failure.

What is rhabdomyolysis?

Rhabdomyolysis, also known as' Haff's disease ', comes from the Baltic Sea coast of Haff, where rhabdomyolysis was first discovered.

Rhabdomyolysis is a disease with unknown pathogenic factors. The cause of rhabdomyolysis is not the rumored 'shrimp washing powder' - oxalic acid. The scientific community can only confirm that the pathogenic factor is a non-polar toxin which is stable to heat and dissolves in n-hexane, not a living pathogenic microorganism.
