Go to the department dealing with your traffic accident to hand in the application for disability identification, obtain the relevant legal documents of the traffic management department with the application, and take the documents (which must be signed and sealed) to the designated judicial identification agency for identification.
Materials to be carried in traffic accident disability identification:
1. The assessed person shall carry the disability assessment application form with the official seal of the handling unit and the signature of the handling person;
2. Carry the diagnosis certificate, examination result, CT, X-ray and diagnosis report of the early stage of injury and the end of treatment;
3. Borrow relevant operation medical records and examination records from the treatment hospital;
4. When assessing the working ability of the dependent, the ID card, household registration certificate of the assessor and the explanation of the relevant government departments shall also be carried;
