1. Passport: original passport and photocopy of photo page. The passport should be valid for more than six months (from the date of entry) and have at least one blank visa page.
2. Form 14a visa application form (original): an application form completed in English and signed by the applicant.
3. Color photos: two (one posted on the form and the other for scanning). Photos shall meet the following requirements:
a) Two inch, color, white background within three months of the latest photo;
