How to distinguish chronic atrophic gastritis and chronic superficial gastritis?

Chronic atrophic gastritis and chronic superficial gastritis belong to one kind of chronic gastritis. The two symptoms are very similar, so it is not difficult to distinguish them. The general difference can only be distinguished by auxiliary examination: chronic atrophic gastritis is characterized by atrophy of the intrinsic glands of the gastric mucosa (decrease in quantity and function), often accompanied by intestinal metaplasia and inflammatory reaction. Chronic atrophic gastritis was divided into three grades: mild, moderate and severe. The reduction of the intrinsic glands of the stomach (the pyloric glands in the antrum, the gastric fundus and the CARDIA) by 1 / 3 is mild, 1 / 3-2 / 3 is moderate, and more than 2 / 3 is severe.

The gastric glands (CARDIA gland, fundus gland and pylorus gland) of superficial gastritis are normal all the time without damage or decrease in number. Superficial gastritis develops further, and its intrinsic glands are reduced due to inflammation, which can be transformed into atrophic gastritis. The clinical manifestations of atrophic gastritis are not only lack of specificity, but also not completely consistent with the degree of pathological changes. The following clinical manifestations often appear: fullness of the epigastric region; pain in the epigastric region; heartburn and dyspepsia; abnormal and weak stool symptoms; anemia.

Generally speaking, the prognosis of chronic superficial gastritis and simple mild chronic atrophic gastritis is good. The main manifestations of chronic superficial gastritis under gastroscope are as follows: ① the gastric mucosa is congested and edematous, and the congested and edematous areas can cross each other to form red and white, and the main color is congested red; ② the surface of the gastric mucosa has sticky gray white or light yellow mucus spots; ③ the gastric mucosa has bleeding spots; ④ sometimes small erosions can be seen on the mucosa. The manifestations of chronic atrophic gastritis under gastroscope are as follows: ① the color of gastric mucosa becomes light, reddish and grayish yellow, and the most severe is gray white or grayish blue.
