5. Layout plan of business place and warehouse and house lease contract (1 copy, check the original) (must be signed by the sponsor and sealed by the housing leasing management department). If the business place and warehouse are self owned property, the house property right certificate shall be submitted (1 copy, the original copy shall be checked);

6. Certification documents of enterprise name issued by the administrative department for Industry and Commerce (1 copy, check the original; for example: business license, notice of enterprise name pre approval, name inquiry certificate, etc.);

7. Health food hygiene and quality management system (1 copy);

Examination and approval of certificate issuing:
