2、 Process:

New license:

1. Application report;

2. Application for health license of Health Bureau of XX city;

3. A copy of the notice of enterprise name pre approval;

4. List of health management organizations (structures);

5. Health management system of various places;

6. List of main production equipment;

7. Construction project layout plan (air conditioning system workshop, production equipment, water supply and drainage system, health facilities layout, ventilation and exhaust), blueprint or computer print (indicating size, area and name of business unit) shall be provided. (the unit that has completed the sanitation acceptance of the construction project in our institute can be exempted from providing);

8. Health certificate and health knowledge training certificate of employees (not required when applying, but must be provided before obtaining the certificate).

Renewal and review:

1. Application report;

2. Application for health license of Health Bureau of XX city;

3. Copy of business license (duplicate);

4. The original copy of the original hygiene license and the original product record form;

5. Copies of health certificate and health knowledge training certificate of employees: all copies shall be provided for less than 10 people, and roster shall be provided for more than 10 people. The list includes: serial number, name, gender, age, type of work, date of issuance of health certificate, certificate number, date of issuance of training certificate, physical examination unit and training unit;

6. The applicant shall submit the changed materials in respect of any change in the production site, process layout, product formula, process and label, sanitary facilities, production equipment and laboratory facilities, air conditioning system, water supply and drainage system, etc;

7. Product hygiene inspection report (within one year).

Change:

1. Application report;

2. Application for health license of Health Bureau of XX city;

3. New products shall be subject to "new product review", and the required data shall be the same;

4. In case of product reduction, only 'application report' shall be submitted;

5. The original copy of the original health permit and the copy of the industrial and commercial business license.
