1、 Object of food hygiene license

Units or individuals engaged in the following food business activities in Shanghai shall obtain a food hygiene license

1. Catering industry;

2, canteen;

3. Food sales (except that the food production enterprises use the production area to open a sales department to engage in sales activities);

4. Food is prepared and sold on the spot;

5. Food storage and transportation;

6. Managers of wholesale and retail markets of edible agricultural products and other food trading markets;

7. Engaging in the following food business activities in food trading markets such as wholesale and retail markets of edible agricultural products;

8. Non direct imported food is on the spot;

9. Non fixed packaging direct import food sales;

10. Fresh frozen animal meat, bean products (including bean sprouts) sales;

11. Engaged in food sales and on-the-spot sales in other commodity trading markets.

2、 Materials required for the application of Shanghai food hygiene license

(1) The applicant shall fill in the corresponding application form and submit the following relevant materials to the acceptance window of Shanghai food and Drug Administration or each district / County branch

1. Application for Shanghai food hygiene license or Shanghai food hygiene license (Provisional) (including new license and extension)

1) Shanghai food hygiene license application;

2) Relevant certificates of legal representative (person in charge, owner), qualification certificate of entrusted agent and other materials;

3) Schematic diagram of places and equipment layout, production process flow and sanitation facilities for food business activities;

4) Preventive health examination materials for new construction, reconstruction and expansion projects;

5) undertaking;

6) Relevant certificates for the legal use of the business site.

When applying for the production of health food, boxed rice or barrel rice (including the production of both the above food and other food), or the food is produced and sold on the spot, the following materials shall also be provided:

1) Product raw material formula;

2) Product packaging form;

3) Product process;

4) Sample labels and instructions;

5) Data of production and processing equipment and facilities;

6) Product hygiene quality standard;

7) A copy of the entrustment contract for the entrusted production and processing of products.

8) The application for health food production shall provide the corresponding product approval certificate (approved by the State Food and Drug Administration or the health department).

3、 Approval process of food hygiene license

1. Shanghai food and Drug Administration or each district and County branch shall conduct formal examination and issue acceptance notice for qualified ones.

2. After acceptance, Shanghai food and Drug Administration or each district / County branch shall assign personnel to conduct data and on-site audit in accordance with the requirements of the food hygiene law of the people's Republic of China.

3. Shanghai food and Drug Administration or each district and County branch shall make the decision of approval (or disapproval) of administrative license based on the audit results. If a decision on approval is made and a health permit is to be issued, a health permit shall be issued to the applicant.

4、 Time required for handling food hygiene license

1. The food and drug administration shall decide whether to accept the application or not within five working days after receiving the application.

2. The examination and approval decision shall be made within 20 working days after acceptance.

3. The food hygiene license shall be issued within ten working days after the approval decision is made.
