The first step is to take your ID card to the city where you worked before and find the service window of XX social insurance fund administration to print such a document.
Step 2, go back to your local social security fund authority and give you two forms.

The first one is easy to handle. Just fill in the basic information.
The second is hard to find on this official seal, which refers to the name of the company that is buying you social security now.
1. If the company that buys social security for you is a local company that has nothing to do with your previous company, it's better to deal with it. Just look for the existing company's finance.
2. If you are still in the previous company, it may be that your company found a local human resources company to buy on behalf of you, and you have to find the finance of the agency. (mine is type 2)
Finally, you can copy an ID card, plus the first three copies of a total of four forms, and give them to your local social security insurance fund management bureau. After three months, you can check the results online.
