The following conditions shall be met for the barber shop to apply for health license:
1. Industrial and commercial name approval;
2. Property right certificate and lease contract of the business site;
3. Health certificate of employees;
4. Health management system of barber shop;
5. The barber shop is equipped with disinfection facilities, such as ultraviolet lamp, disinfection cabinet, etc;
6. After the special tools for skin diseases meet the above requirements, they shall apply to the local health department for a public place health license.
How to apply for a health license:
First go to the local CDC for physical examination and apply for the health certificate (the physical examination fee and one inch photo are required), and then go to the local health supervision institute to apply for the health knowledge training certificate (the training fee and one inch photo are required). After that, the name approval will be carried out in the local industrial and commercial department, and the enterprise name approval notice will be obtained, together with a copy of the ID card of the person in charge, a copy of the health certificate and training certificate, a copy of the plan of the business place, a copy of the facade leasing contract of the business place, and a copy of the marriage and childbearing certificate of the floating population.
