1、 The importance of birth medical certificate

1. To prove the health and natural condition of the newborn at birth;

2. To prove the consanguinity of the birth population;

3. As the medical basis for the acquisition of nationality;

4. As the medical basis for the registration of the birth population by the household registration authority;

5. As evidence of the newborn's access to preventive health care services in accordance with the law;

6. Valid certificates that must be provided for studying abroad, immigration, etc.

2、 Materials to be submitted for the first application for birth medical certificate

1. Medical record of newborn birth issued by delivery institution;

2. The original and copy of valid identity documents of both parents of the newborn;

3. The birth medical certificate must be applied for by the mother of the newborn. If special circumstances can be handled by the father or other guardians of the newborn, but a power of attorney signed by the mother must be provided.

3、 Handling of newborn birth certificate

1. Take the name of the newborn in advance. A newborn can take his father's or his mother's surname. The use of names must be in accordance with the Chinese character list of the general specification, and no unsociable or traditional characters shall be used. Once the birth medical certificate is confirmed and printed, the name of the newborn cannot be changed.

2. The parents of the newborn must provide the original and copy of their valid identity documents, and shall not borrow or forge identity documents, otherwise the legal disputes or inconveniences caused thereby shall be borne by the parents of the newborn.

3. The birth medical certificate shall be completed within one month after the birth of the baby, and shall not exceed six months at the latest due to special circumstances. Delaying the application of birth medical certificate will bring many unnecessary troubles to the parents or guardians of the newborn. 5. The medical certificate of birth consists of front page, back page and stub. The front page shall be properly kept by the parents or guardians of the newborn for life. The back page shall be submitted to the household registration authority as the certificate of birth registration, and the stub shall be filed by the issuing authority. Before going through the household registration, the front page and the back page shall not be removed without permission, otherwise they will become invalid certificates.

6. After the birth medical certificate is handled, it shall be properly kept for life, and shall not be lost, damaged, altered, and stored in a dry place to avoid moisture.

4、 First application for birth medical certificate under special circumstances

1. Handling of births outside medical institutions. Babies born outside the medical institutions (including those born at home, on the way and in institutions without midwifery qualification) must go to the County Maternal and child health care center. In addition to providing the original ID card and household register of both parents and the police station's certificate of not going to the house, they must also submit the DNA parent-child relationship judicial appraisal certificate issued by a qualified appraisal institution, the "parent-child relationship statement" signed by the parents of the babies, and the "medical machine" The delivery form of the newborn born outside the organization must indicate the basic information of the delivery personnel, the signature of the delivery personnel and the delivery witness, and also provide the copy of the effective ID card of the delivery personnel and the delivery witness.

2. Single parent processing. 3. Single parent processing. Five, renewal

(1) If the birth medical certificate is invalid for some reason, or the birth population registration cannot be carried out, an application for renewal shall be made to the original issuing unit. If the birth registration has been completed, the birth medical certificate shall not be renewed. Information to be submitted for renewal:

1. The original and copy of valid ID card or passport and household register of the newborn's parents;

2. Statement of parent-child relationship issued by the parents of the newborn;

3. The original of the first page and the second page of the original issued birth medical certificate;

4. Copies of medical record of newborn birth and original medical record (including the first page of inpatient medical record, discharge record and delivery record) issued and sealed by the original delivery medical and health care institution.

5. In addition to the above information, a DNA paternity forensic certificate issued by a qualified parent-child appraisal institution must also be provided for the replacement of medical institutions born outside the home, on the way and without midwifery qualification.

(2) . if the party concerned requests to change the relevant information of the newborn or its parents after providing false information to obtain the first issued birth medical certificate when applying for the first issuance of the birth medical certificate, he shall apply to the County Maternal and child health center for the renewal of the birth medical certificate, but the natural condition items of the newborn shall not be changed. Six, Reissue

If the birth medical certificate is required to be reissued due to loss, theft and other reasons, it must be applied for reissue at the County Maternal and child health care center. The following information shall be provided for reissue:

1. The original issuing institution shall issue a copy of the stub of the birth medical certificate;
